In 2018 a group at the Cleveland Clinic followed 122,007 patients through a treadmill test and found that the least fit among them were dying at five times the rate of the fittest - a gap wider than smoking, wider than diabetes, wider than diagnosed coronary disease. Within five years that finding had become the most confident claim in consumer health: that a single number predicts how long you will live, and that raising it will extend your life.
The finding is real. Almost everything built on top of it is not.
The VO₂ Max Protocol goes back to the papers underneath the claim and reports what they actually measured. The Cleveland study never measured VO₂ max at all; it estimated exercise capacity from treadmill speed and grade, with nobody wearing a mask. When researchers used inherited genetic variants to test whether fitness causes longevity, they found no causal path.
None of which is an argument against training. It is an argument for knowing what you are holding. What emerges is smaller than the marketing and considerably more useful: about three to four hours a week of aerobic work, drawn from the one randomised trial that assigned exercise and counted deaths, with resistance work beside it and no target number to chase. The steepest returns belong to the person currently doing nothing, which is the reverse of where the entire industry aims.
